Mandatory Requirements
- 1. It is mandatory for the suspect patient to Show the valid doctor’s prescription stated the requirement of the test.
- 2. As per the Ministry of Health and Family Welfare (MoHFW) guidelines, Government ID ( Aadhaar card / Voter Id / Passport)/Address Proof to support the current address and contact number of the suspect case should be given at the time of sample collection.
What Is a Valid Doctor’s Prescription?
The prescription should mention the requirement of the test and should have the Doctor’s medical registration number and Doctor’s stamp.
This booking is subject to the fulfillment and verification of mandatory requirements by Third Party Labs. Third-Party Labs will verify the mandatory requirements forwarded by you and in case of Third-Party Lab(s) observe any discrepancy, the Third-Party Lab (s) will cancel the order immediately.
